  Swetabh Verma The pt has a kyphotic spine with loss of lordosis..it is not expected to fall back and provide indirect decompression by laminoplasty/laminectomy..moresoever the compression is anterior. Two level acdf c3-c4& c4-c5 with tricortical iliac crest graft/cage with anterior cervical plate would b good..or a corpectomy c4 with decompression at above and below level with grafting can also b done

  Ashish Agarwal open ant. , decompression two level c3-c4 and c4-c5.if diameter is reduced then do laminectomy and fixation with plate.
